You will need the following information to complete the form:

 

1. A Google account (free to register if you don't have one or you can email us)

2. Your pet's records from your previous veterinary hospital

Steps to complete the form:

1. Click on the button "Register your pet(s)"

2. Enter your pet's details, one at a time

3. Upload their medical records

4. Repeat for additional pets
